MEDICATIONS
Parents must sign an authorization form in the Health Clinic for any medication to be administered at school.  Prescription medication will be given to a student only if the medicine is contained in a prescription vial with a label affixed with the name and address of pharmacy or origin, date, name of patient, doctor's name, directions for administration and prescription number.

ILLNESS
If your child becomes ill or is injured at school, they will be cared for temporarily and you will be notified.  Please keep telephone numbers and addresses updated so you can be reached quickly.
